Paradigm Personnel and Applicant Privacy Notice

Paradigm Management Services, LLC and its affiliates and subsidiaries, including Foresight Medical, LLC, Adva Holdings, LLC, Encompass Specialty Network, LLC, Adva-Pac, LLC, Alaris Encore Minnesota, LLC, The Alaris Group, Inc., Encore Unlimited, LLC, Encore Alliance, LLC, HomeCare Connect, LLC, and Welvie, LLC (“Paradigm”) has developed this Personnel and Applicant Privacy Notice (“Notice”) to help our prospective and current employees and contractors understand the personal information we collect, use, and share about you. For California residents, this notice also describes rights available to you under the California Consumer Privacy Act (“CCPA”).

In addition to personal information we collect about you, this Notice applies to any personal information that we may collect about your emergency contacts. For current employees, this Notice additionally applies to personal information we may collect about other individuals to provide employment benefits. We refer to all individuals covered by this Notice as our “prospective and current employees and contractors” within this Notice.

Information Collection

In the past 12 months, we collected and disclosed the following categories of personal information about our prospective and current employees and contractors. We may also collect and disclose these categories of personal information going forward:

Personal and online identifiers, such as first and last name, email address, Social Security number, and driver’s license or state identification number.
Recordkeeping information, such as insurance policy number, bank account number, and medical information, including vaccine records and drug test results if applicable.
Characteristics of legally protected classifications, such as race, gender, sex, or marital status (including for administering employee benefits).
Commercial or transactions information, such as expense records.
Internet or other electronic network activity information, such as browsing history, search history, and interactions with an application, website, or email.
Audiovisual information, such as call or videoconference recordings or other audio, electronic, visual, or similar information.
Professional or employment-related information, such as records relating to your prospective or current employment with Paradigm, your employment history, and documents for employment eligibility.
Education information, such as professional training records, information included on your resume, and your education history and transcripts.
Sensitive personal information, such as passport information for eligibility purposes, health information about current employees and contractors for disability accommodations or benefits, , in addition to other data types described above.
Other information about you that is linked to the personal information above, such as date of birth.

Categories of Sources

We collect personal information about prospective and current employees and contractors from the following categories of sources:

From you. We collect personal information directly from you during your relationship with Paradigm. We also may collect personal information about you indirectly, such as by monitoring your activities in accordance with our policies and procedures.
Paradigm personnel. When you are an emergency contact or beneficiary of a Paradigm employee or contractor, we may collect personal information about you from that employee or contractor. When you are a Paradigm employee or contractor, we may collect personal information about you from other Paradigm personnel, like your manager. When you apply to a position at Paradigm, we also may collect personal information about you from other Paradigm personnel, including those who interview you.
Our service providers. We collect personal information from our services providers, such as those who help process job applications and those who administer payroll and employee benefits. We also may collect personal information from our service providers offering artificial intelligence tools that support our business.
References. We may collect personal information from references you provide when you apply to a position at Paradigm.
Recruiters. We may collect personal information from individuals that refer you to positions at Paradigm.
Social media companies. We may collect personal information available on social media pages.
Educational institutions. We may collect personal information from educational institutions, such as to obtain or confirm transcripts about prospective employees or contractors.
Background check providers. We may collect personal information from background check providers, including consumer reporting agencies, if we obtain a background check about you.

For current Paradigm employees and contractors, we also may collect personal information about you from Paradigm clients, including when clients provide feedback about your performance.

Using Personal Information

We use and disclose the personal information we collect about prospective and current employees and contractors for our commercial and business purposes. These commercial and business purposes include, without limitation:

For prospective employees and contractors, processing, evaluating, and communicating with you about your application, including to check references, conduct background checks, and communicate with you about other positions that may interest you.
For current employees and contractors, using your information in connection with your professional roles and responsibilities. This includes, for example:
- Providing you with services and tools to use, such as email service and information technologies, in connection with your relationship with Paradigm;
- Responding to your requests;
- Maintaining compliance with our policies and procedures;
- Paying you and administering any employment benefits you may receive; and
- Responding to emergencies or safety concerns, such as by maintaining and using your emergency contact information.
For both prospective and current employees and contractors, other business purposes as identified in applicable laws, which include:
- Auditing and debugging;
- Legal compliance;
- Detecting and protecting against security incidents, fraud, and illegal activity;
- Performing services like account servicing and analytics;
- Internal research for technological improvement;
- Internal operations;
- Activities to maintain and improve our business; and
- Other one-time uses.

Sharing Personal information

We do not sell the personal information we collect about prospective or current employees or contractors or share this information for online targeted advertising purposes (also known as “cross-context behavioral advertising”). However, we disclose the personal information we collect to the following categories of recipients:

Service providers. We disclose personal information to our service providers, such as those that help us manage our job application processes, maintain our records, administer employee benefits or payroll, or assist with human resources functions. We also disclose personal information to service providers that provide artificial intelligence tools, including tools that help us record and summarize interviews and other meetings.
Paradigm affiliates. We may disclose personal information within Paradigm, including to Paradigm affiliates and subsidiaries.
Recruiters. We may disclose personal information to recruiters, such as to convey the status of your application or to confirm the status of your employment with Paradigm.
Employment references. We may disclose personal information to references you provide, such as to confirm the information you provide when applying to a position at Paradigm.
Background check providers. As legally permitted, we may disclose personal information to background check providers to obtain a background check.
Government entities. We may provide personal information to government entities, such as for reporting purposes.
To third parties when legally required. We may also provide personal information to other third parties in response to legal process, for example, in response to a court order or a subpoena.

For current Paradigm employees and contractors, we also may disclose personal information to Paradigm clients to provide our services.

Retaining Personal information

The personal information we collect is stored by Paradigm or our service providers on our behalf. We retain personal information that we collect for as long as we need to use it in connection with our business and in accordance with applicable laws. To determine how long we retain personal information, we consider the amount, nature, and sensitivity of the personal information, the potential risk of harm from unauthorized use or disclosure of personal information, the purposes for which we process personal information and whether we can achieve those purposes through other means, and the applicable legal requirements.

Your rights

California residents who are our prospective or current employees and contractors have certain rights under the CCPA related to personal information we collect. If you are a California resident, you may have the following rights, subject to certain exceptions and limitations:

The right to know the categories and specific pieces of personal information we collect, use, and disclose about you, the categories of sources from which we collected personal information about you, our purposes for collecting or disclosing personal information, and the categories of third parties to which we have disclosed personal information.
The right to request that we delete the personal information we have collected from you.
The right to correct inaccurate personal information we maintain about you.

You also have the right not to receive discriminatory treatment for the exercise of the privacy rights conferred by the CCPA.

The CCPA also provides an opt-out opportunity related to sensitive personal information. However, Paradigm uses and discloses sensitive personal information, as defined under the CCPA, about our prospective and current employees and contractors only for purposes permitted by the CCPA that do not require an opt-out opportunity.

Verification Process and Required Information. In some cases, we may need to request additional information from you to verify your identity or understand the scope of your request. Generally, we verify your identity by matching the information provided in your request with the information we maintain in our records.

Authorized Agent. You may designate an authorized agent to submit a request on your behalf. To designate an authorized agent to make a request on your behalf, you must provide the agent with signed permission to do so and provide proof of your identity, or the agent must have a valid power of attorney. An authorized agent can make a request on your behalf through one of the submission methods noted above. For us to process the request, the authorized agent will be required to provide evidence of signed permission and your e-mail address to verify your identity, or proof of valid power of attorney.

Changes to This Notice

If our information practices change, we will update this Notice and post the changes on this page. We encourage you to visit this page periodically to learn of any updates.
